A short, plain-language account of the information this website collects, who reads it, and how long it is kept.
- What this page covers
This policy describes how Develop Medical Group handles information submitted through this website. It does not describe the handling of your medical record once you are a patient; that is governed separately by our HIPAA Notice of Privacy Practices, which you receive when you join the practice.
- What we collect
Through the membership request form we collect your name, email address, optional phone number, the membership you are considering, and anything you choose to write in the free-text field about what you are hoping to change.
We ask you not to include detailed medical information in that field. A short sentence about your goals is enough for an introductory conversation.
- How we use it
We use what you send only to contact you about becoming a patient and to prepare for that first conversation. We do not sell it, rent it, or share it with advertisers, and we do not use it for marketing lists.
- Where it goes
Form submissions are delivered to the practice's own inbox and are read by Dr. Augdahl. Access is limited to the physician and any staff directly involved in patient intake.
- How long we keep it
If you become a patient, the request becomes part of your intake documentation. If you do not, we delete the request within twelve months.
- Your choices
You can ask us at any time to tell you what we hold, correct it, or delete it. Email caugdahl@developmedak.com and we will act on the request.
- This website
The site does not use advertising trackers or third-party analytics profiles. Any cookies present are those required for the site to function.
- In an emergency
This website is not a channel for urgent medical concerns. If you have one, call 911 or go to the nearest emergency department.
